How can I add and design a form?

Select [+Add new form] in the top left corner.


Select [+Add form template].


You can choose to custom the form or choose from template.


After selecting [Custom], you can start designing your own form. Don't forget to go to "Work Flow," "Permission," and "Misc" to change those settings as well.
